If you would like to become a chaperone and fall under Staffordshire County Council please complete the 3 forms below and attach above/ email to scanedance@gmail.com with a passport sized photograph of yourself (this can be a selfie taken at home as long as there is nothing in the background

and your full face is visible).

Please note that if you already hold an Enhanced DBS that is dated within the last 12 months when applying  you DO NOT have to complete the DBS Application Form (1), attach a copy of your Enhanced DBS instead.
